Residential Services

 The Jefferson Rehabilitation Center offers several residential opportunities for individuals with developmental disabilities.  The type of setting is based upon a person’s particular need, preferences and availability.  Varied opportunities are provided for skill development in areas such as socialization, activities of daily living, med administration, self care skills, community integration and meal preparation.  The residents take an active role in the maintenance of the site and assist in everything from meal preparation to selecting furniture.  There are several housing models:  Intermediate Care Facilities (3) are a medical model that meets the needs of residents with medical and clinical needs, Supervised Individual Residential Alternatives (25) range in size from 2 beds to 11 beds and provide 24 hour supervision and varied services as described above, Supportive Individual Residential Alternatives (3) provide services specific to each person and average about 12-15 hours a week of staff support and a Community Residence that has 13 residents and provides 24 hour supervision and a variety of skill based services and supports.  In addition we offer respite services at two locations and provide residential and day hab services at home to three people in the community.

The Residential Department has continued to emphasize community involvement and inclusion and has also concentrated on having the residents involved in hiring, policy and procedure decisions and advocacy.  Our Residential Advisory Board is very involved in making recommendations, providing input on any changes being considered and in interviewing and selecting candidates.  They are also very involved in the self advocacy movement and many belong to the North Country Self Advocates.  Several have also traveled to Albany for conferences and to meet with the Senator and Assemblywoman to discuss issues that impact them in relation to the NY State budget.
